Welcome to the second edition of the Incident AI Newsletter – your quick update on how Incident AI is evolving to make your investigations faster, sharper, and more reliable.

We’ve been listening closely to feedback from users like you. This month, we’re excited to introduce three powerful tools now available in Incident AI. Our current subscribers can now have extensive timeline/5why investigations and also output their final outputs in beautifully crafted presentations made by our powerful AI tool.

  1. Timeline/Why - Major Upgrade

We heard your feedback that majority of your investigations are done on Timeline/Why level, so we have invested a lot of time to ensure you can get the most out of it in Incident AI.

What’s new

+ WAD vs WAI first. We check Work as Done against Work as Intended before asking “why.” Gaps show up straight away.

+ No proof = “Unknown.” If we don’t have evidence that something contributed, it shows Unknown. That’s your cue to collect what’s missing (photos, statements, logs, etc...).

+ Every Why has a source. Each Why includes a short evidence tag so you can see where the answer came from.

+ Stops when the evidence stops. The chain ends when proof runs out—cutting opinion and AI bias.

+ CTRL tags for controls. The tool reads your uploaded SHMS/safety docs and drops in CTRL: notes (e.g., Preventive/Detective) so critical controls and gaps are visible in-line.

+ Evidence list at the end. Each row ends with a quick reference list for audit and sign-off.

Why this helps on site

+ Faster reviews with clearer actions.

+ Precise follow-ups on the real gaps.

+ A traceable record that stands up in audits and investigations.

Timeline + 5-Whys is the standard across most companies—that’s why we’re committed to getting the best possible outcome from our models.

   2. Generate Investigation Output Presentation - Almost Magic

Turn approved investigation outputs into redacted, consistent presentation packs in ~70 seconds—ready for toolbox talks, leadership briefings, and board updates.

Why it matters

Speed: hours saved on formatting and redaction

Quality: clear narrative aligned to your method (ICAM/HOP/CCM)

Governance: no sensitive data; content traceable to approved outputs

Here are some sample outputs:

Our current subscribers can access this feature now, free of charge. Contact us for more information, or if you would like to conduct a free trial.

We will also contact you in the next stage of this project to discuss if you would like to opt in for industry wide sharing (2pagers) across our subscribers.

    3. Privileged Investigations - In support of your legal house

We know offer solutions for companies that are willing to enable privileged investigations on our platform. Privileged events are only allowed to be accessed by the creators. Incident AI displays your custom disclaimer on our UI, and pdf report at every page. Moreover, the pdf report is encrypted and password protected. Please contact us if you would like to enable this feature as part of your subscription, free of charge.

Why these matter:

Together, these upgrades matter because they compress the time between incident and learning, replace opinion with evidence, and strengthen governance. By checking WAD vs WAI first, handling Unknowns explicitly, and citing the source for every Why, Incident AI focuses teams on the real gaps and the critical controls tied to your SHMS, bow ties, and PHMPs (ICAM/HOP/CCM). One-click presentation generation turns approved outputs into redacted, consistent decks in ~70 seconds—making toolbox talks and leadership briefings engaging and audit-ready—while privileged mode and encrypted PDFs protect sensitive matters. Most importantly, humans stay in control—your team approves and edits—so the time saved goes into closing actions and preventing repeat events.

What’s next:

We’re working on the next set of improvements, including:

  • An interview voice recorder as an optional addition to the Interview Questions tool: collect evidence fast!

  • A photo library that lets you prepare incident briefs and outcome summary reports instantly.

Want to try these new tools?

If you’re currently trialing Incident AI, you’ll see them in your dashboard now. If you’ve enquired but haven’t started a trial, feel free to contact us.

Here’s to better, faster, more defensible investigations.
